Sputum is matter that is expelled from the respiratory tract, such as mucus or phlegm, mixed with saliva, which can then be spat from the mouth. It is usually associated with air passages in diseased lungs, bronchi, or upper respiratory tract. It can be found to contain blood if in a chronic cough possibly from severe cases of tuberculosis.

A sputum sample is the name given to the mucus that is coughed up from the lower airways. It is usually used for microbiological investigations of respiratory infections.

The best sputum samples contain very little saliva, as this contaminates the sample with oral bacteria. This event is assessed by the clinical microbiologist by examining a Gram stain of the sputum. More than 25 squamous epithelial cells at low enlargement indicates salivary contamination.

When a sputum specimen is plated out, it is best to get the portion of the sample that most looks like pus onto the swab. If there is any blood in the sputum, this should also be on the swab.

Microbiological sputum samples are usually used to look for infections by Moraxella catarrhalis, Mycobacterium tuberculosis, Streptococcus pneumoniae and Haemophilus influenzae. Other pathogens can also be found.

Purulent Sputum is that containing, or consisting of, pus.

Sputum can be:

  1. Bloody (often found in tuberculosis) (Haemoptysis)
  2. Rusty colored - usually caused by pneumococcal bacteria (in pneumonia)
  3. Purulent - containing pus. The color gives hints about probable causes:
    1. a yellow-greenish (mucopurulent) color points to a bacterial infection.
    2. a white, milky, or opaque (mucoid) appearance often means a viral infection.
  4. Foamy white - may come from obstruction or even Edema

Does anyone know if MRSA in a persons sputum can infect people caring for the sick person?
Q. I just started working in a nursing home last month and one of the people living there have MRSA in their sputum. I was told to wear a mask by coworkers...however the D.O.N. said I was silly to be scard. I have a small daughter at home and don't want to bring anything to her.

A. Generally this can be done by a respiratory practitioner. They give you a warm mist to breathe which makes you cough & anything you bring up from deep in your lungs you caough into a cup (NO saliva or spit); that then goes to the mircobiology dept at a lab and they culture it onto different medias (like a special growth media for TB) and do a gram stain and a acid fast stain & look at the results under a microscope to see if harmful bacteria are there. It can take quite a time for TB to grow (weeks) so the final results take a long time.

A. Well, if you just spit in the collection cup, then we will reject the specimen. The lab has certain criteria for an acceptable sputum specimen, which we determine by doing a gram stain (slide stained to observe cells and bacteria). If there are >25 epithelial (skin) cells, <25 white blood cells, and little to no mucus we will request another specimen. If the specimen is acceptable and for tuberculosis, we will collect three specimens over a three day period. Also, the best sputum specimen is a first morning, and, if you are an inpatient in a hospital, one induced by a respiratory tech. Bottom line, if you need to give a sputum culture, cough from your toes and give us a good one!
